Is there a lot of items and lots of folders in the delegate mailbox? I've seen major sync issues in the past with shared mailboxes when there are over a certain number of folders, and some of the folders then stop syncing properly. This was in Outlook 2016. Microsoft acknowledged the issue in a support case and we tried temporarily converting the mailbox to a user mailbox with a licence and logging into it inside Outlook properly (no delegation) which fixed the issue.

I.e. add an account and get the manager to type their credentials instead of relying on delegated access as a test. It may be easier to do this under a separate Outlook profile.
